Background The outcome of Systemic Lupus Erythematosus(SLE) pregnancies has dramatically improved thanks to pregnancy planning, multidisciplinary management and close monitoring during pregnancy. In our experience, programmed pregnancies in SLE patients had similar rates of pregnancy losses as compared to general obstetric population, but there are still open issues on some pregnancy complications that more frequently affect SLE patients. Objectives To analyse the obstetric outcome of SLE patients, according to specific therapy received during pregnancy. Methods A monocentric, retrospective study of 98 SLE patients with a total of 134 pregnancies followed prospectively by multidisciplinary team(1987–2015). Adverse Pregnancy Outcomes(APOs) were defined as one of the followings:premature miscarriage(<10 th week), intrauterine fetal death(>10 th week), perinatal death(<30 th day of life), severe preterm birth(<34 th week) and preterm birth(between 34th-36th weeks). We also evaluated the frequency of other pregnancy complications such as preterm premature rupture of membranes(pPROM) and pre-eclampsia(PE). Results Among the 134 pregnancies(including 3 twin pregnancies), flares occurred in 10 (7.5%) and APOs in 39 (29.1%) cases (table 1). pPROM complicated 9 pregnancies, all resulted in preterm deliveries, including 3 severe preterm birth; PE complicated 6 pregnancies resulting in 2 preterm birth, 1 intrauterine fetal death, 1 perinatal death and 2 term birth. The rates of APOs, pPROM and PE were compared according to receiving or not a specific therapy: hydroxychloroquine(HCQ), low dose aspirin(LDA), immunosuppressant(IS) during the overall pregnancy and corticosteroids>35 mg/week(CS) during the 1 st, the 2nd and the 3rd trimester. No statistical significant association was found between a specific therapy and the rate of PE. AbstractBackgroundPosttraumatic stress disorder (PTSD) is heritable and a potential consequence of exposure to traumatic stress. Evidence suggests that a quantitative approach to PTSD phenotype measurement and incorporation of lifetime trauma exposure (LTE) information could enhance the discovery power of PTSD genome-wide association studies (GWASs). MethodsA GWAS on PTSD symptoms was performed in 51 cohorts followed by a fixed-effects meta-analysis ( N = 182,199 European ancestry participants). A GWAS of LTE burden was performed in the UK Biobank cohort ( N = 132,988). Genetic correlations were evaluated with linkage disequilibrium score regression. Multivariate analysis was performed using Multi-Trait Analysis of GWAS. Functional mapping and annotation of leading loci was performed with FUMA. Replication was evaluated using the Million Veteran Program GWAS of PTSD total symptoms. ResultsGWASs of PTSD symptoms and LTE burden identified 5 and 6 independent genome-wide significant loci, respectively. There was a 72% genetic correlation between PTSD and LTE. PTSD and LTE showed largely similar patterns of genetic correlation with other traits, albeit with some distinctions. Adjusting PTSD for LTE reduced PTSD heritability by 31%. Multivariate analysis of PTSD and LTE increased the effective sample size of the PTSD GWAS by 20% and identified 4 additional loci. Four of these 9 PTSD loci were independently replicated in the Million Veteran Program. ConclusionsThrough using a quantitative trait measure of PTSD, we identified novel risk loci not previously identified using prior case-control analyses. PTSD and LTE have a high genetic overlap that can be leveraged to increase discovery power through multivariate methods.

Background: It is currently not known if TNF or IL-17A inhibitors have an impact on immune cell frequencies in axial Spondyloarthritis (AxSpA). This question is important to understand the impact of biologics on the immune system. Data from clinical trials didn't show significant modification on immune cells and especially on lymphocytes. But regarding the risk of infections linked to these treatments lymphocyte cell subsets are certainly disturbed. Moreover, biologics could affect subsets of cells with an unusual phenotype. Objectives: To identify the phenotype of cell subsets affected by biologics. Methods: We used an “unsupervised approach” to analyze CD4+ T cells and CD8+T cells subsets. Contrary to a “supervised approach”, this strategy takes advantages of the fluorescence emitted by all of the surface markers used to characterize the cells at the same time. The objective was on the one hand to overcome statistical problems related to the number of patients and the repetition of the tests and on the other hand to increase the sensitivity of the analysis by identifying and analyzing new cell populations. The first step was to cluster the cells based on a selection of 12 T cells markers characteristic of the classical cell subsets and the stage of maturation to obtain cell clusters with a phenotype based on the combination of these 12 markers. Then, we were able to describe “a posteriori” the change of frequency of the clusters identified. The second step was to create a visualization of the cells affected to confirm their existence in a classical flow cytometry gate. With this pipeline, we analyzed CD4 and CD8 T cells isolated from a group of AxSpA patients (n=7) before and after 3 months of TNF therapy and a group of patients (n=6) before and after 4 months of IL-17A therapy. ABSTRACTIntroductionIn recent years, many efforts in clinical and basic research have focused on finding molecular features of tumour samples with prognostic or classification value for a given cancer type. To obtain reliable results it is necessary to have good quality input data and use correct statistical methods that support the desired interpretations. The use of combined data from pathways or gene sets to measure a feature of interest is justified by biological and statistical reasons. Material and methodsWe investigate the association between gene expression and probability of survival for colorectal adenocarcinoma (CRC) and breast cancer (BRCA) using a multivariate proportional hazard model. We generate the distribution of Hazard Ratios (HR) from random signatures of increasing numbers of genes for the simplest cox model and for models including different covariables. Results and discussionsOur results show that the estimated (log) Hazard-Ratio (lHR) for the association of the expression of a gene signature with risk of relapse is biassed depending on the cohort under study. More over, this bias seems to be the result of a combination of unknown technical artefacts, biological effects and specific gene-to-gene correlation structures, it depends on the size of the signature and may result in both over and under estimation of the risk. By including covariables that capture global technical effects in the expression matrix we are able to reduce the biases observed using random signatures and to correctly interpret the significance of the associations resulting from the asymptotic test. This can be done as (under certain assumptions) we recover the expected variance of the null distribution by breaking the correlation structure among random signatures induced by the common technical direction.
