Introduction L’efficacité et les modalités d’utilisation des traitements immunomodulateurs systémiques de l’eczéma atopique sont peu étayées scientifiquement. Plusieurs traitements sont utilisés hors AMM (méthotrexate, azathioprine). Les études observationnelles de registres fournissent des éléments indispensables, non disponibles dans les essais randomisés, permettant d’évaluer le rapport bénéfice-risque et le rapport coût-bénéfice des traitements en vie réelle. L’harmonisation des données collectées dans les registres permettra à l’avenir la comparaison et la mise en commun des résultats. L’objectif de ce travail était d’établir un ensemble d’items de base pour ces futurs registres par consensus international. Matériel et méthodes Cette étude a utilisé la méthodologie d’obtention de consensus e-Delphi selon un protocole pré-établi déposé. Résultats Au total, 479 personnes de 36 pays ont accédé à la plate-forme eDelphi, dont 86 %, 79 % et 74 % ont complété les 1 er , 2 e et 3 e tour, respectivement. Les participants comprenaient 6 groupes : 302 (73,7 %) médecins, 21 (5,1 %) infirmières, 52 (12,7 %) patients, 16 (3,9 %) chercheurs, 14 (3,4 %) représentants de l’industrie et (1,2 %) représentants des organismes de réglementation. Lors d’une réunion de consensus à laquelle participaient 42 participants de 13 pays dont 11 européens, le contenu type du registre était établi, contenant 19 domaines et 68 items de base (dont 50 pour la visite d’inclusion et 18 pour les visites de suivi). Discussion Les limites de cette étude sont la sur-représentation des pays européens, des médecins dans les 6 groupes et la barrière de la langue (le questionnaire était en anglais). Conclusion Nous recommandons que cet ensemble d’items de base établi de manière consensuelle à l’échelle internationale soit utilisé pour la constitution des futurs registres nationaux de patients atteints d’eczéma atopique nécessitant un traitement immunomodulateur systémique.

Evolution of patient safety culture in Belgian acute, psychiatric and long-term care hospitals

In Belgium, the federal government launched a national program to support hospitals for implementing quality and patient safety strategies. One of the main objectives in the federal program is the development of a safety culture. The purpose of this study was to examine to what extent the hospitals’ safety culture evolved after participating in the federal program and to explore predictor variables of safety culture. In a cross-sectional follow-up design, safety culture was measured in the Belgian acute, psychiatric and long-term care hospitals using validated translations of the Hospital Survey on Patient Safety Culture in Flemish and French. For both nationwide measurements, hospitals were invited to participate in a benchmark research organized by an academic institution (in 2008 and 2012). Generalized Estimating Equations models were fitted to examine the effect of predictor variables on safety culture perceptions. The Belgian safety culture database contains data from 115 827 respondents from 176 hospitals. For 111 hospitals that participated in both benchmarks it was possible to calculate changes in safety culture. The response rate for the second measurement (52.2%) was comparable to the response rate in the first measurement (51.0%). Improvements were observed for most safety culture dimensions with a major significant improvement for ‘Management support for patient safety’. Although ‘Handoffs and transitions’ and ‘Frequency of events reported’ were key areas within the federal program, a decline was observed for these dimensions. Work area, staff position, language (regional context of hospital), hospital type and hospital statute were found to have important effects on safety culture perceptions. Hospital size and work experience, showed to have less effect on safety culture scores. Large comparative safety culture databases allow identifying patterns and trends. BACKGROUND iMB has inferior survival to older children principally due to radiation-sparing therapy. To better understand which patients may benefit from radiation-sparing protocols, we describe the molecular landscape of iMB and report the iMB outcome on the SJYC07 trial designed to defer, reduce, or delay radiation exposure. METHODS We assembled a molecular cohort of 190 iMBs and a SJYC07 trial cohort of 81 iMBs. Tumors were sub-classified into molecular subgroups based on DNA methylation profiles and overlaid with mutations and copy-number alterations. PFS and OS for the SJYC07 cohort was estimated across clinical risk groups, consensus molecular subgroups, and in the context of novel MB subtypes. RESULTS Computational analysis of DNA methylation array data divided iMB into three of the four consensus subgroups: SHH, G3, and G4 (absent WNT). Clinical outcome of iMB SHH was superior to iMB Group3/Group4 (5-year PFS: 51 ± 8% vs 11 ± 10%, P<0.001; 5-year OS: 72 ± 8% vs 51 ± 12%, P<0.05). t-distributed stochastic neighbor embedding (t-SNE) analysis recognized two subtypes of iMB SHH (iMB SHH-I and iMB SHH-II ) and distributed iMB Group3/Group4 into eight recently described subtypes (I-VIII). 5-year PFS of iMB SHH-II surpassed iMB SHH-I (75 ± 10% vs 28 ± 10%, P=0.003) and exceeded 90% in low-risk iMB SHH-II (<3y, M0, R0, DN/MBEN). CONCLUSION Through integrative genomics, we described the molecular landscape of iMB. Application of these data to a trial cohort identifies: a low-risk SHH-subtype (iMB SHH-II ) that exhibits excellent PFS in the absence of radiation, intra-ventricular chemotherapy, and high-dose chemotherapy; a high-risk SHH-subtype (iMB SHH-I ); and very poor PFS for iMB Group3/Group4 . These findings will shape risk stratification on future iMB trials.
