Accurate estimation of Cobb angle is crucial for clinical diagnosis and treatment of spinal scoliosis. In recent years, there has been significant development in the automatic measurement of Cobb angle using deep learning methods. However, the low contrast of scoliosis X-ray images poses challenges for reliable landmark detection. In this paper, we propose a landmark dectection network of adolescent idiopathic scoliosis by integrating the image enhancement module and vertebral center guidance. In this network, we first utilize an image enhancement network to enhance the overall edge details of the image. Then, by locating the center points of the vertebrae to identify the sequence of vertebrae, it innovatively combines ResNet34 and U-Net architecture, fully integrating deep features with shallow features for landmark prediction. Additionally, we introduce a Global Spinal Loss (GS-Loss) to focus the network more on the spine-related region by considering the spatial characteristics of the spine in the image. Comparative results on the publicly available AASCE challenge dataset demonstrate the superior performance of our method in landmark detection and Cobb angle measurement on X-ray images with low contrast or blur. © 2024 IEEE.

Surgical phase recognition plays a crucial role in computer - assisted surgery systems. Its main purpose is to identify which stage of the surgery the video frames represent. In recent years, remarkable progress has been made in identifying surgical phases using deep - learning methods. However, existing methods mainly focus on improving the accuracy and precision of phase recognition, often neglecting the semantic consistency and temporal continuity of video frames within a phase. This easily leads to prediction errors for blurred and similar frames within a phase, posing challenges to reliable phase recognition. To overcome this problem, we propose a novel method for surgical video phase recognition and construct a temporal model combined with surgical phase localization. By predicting the start and end time points of surgical phases, the model improves the prediction consistency within surgical phases. The model consists of a two - stage training process. In the first stage, the spatial features of the video frame sequence are extracted. In the second stage, a temporal network is first used to perform frame-by-frame surgical phase prediction and surgical phase localization respectively. Finally, the prediction results of frame-by-frame surgical phases and the results of phase localization are fused to generate the final prediction information. This method effectively addresses the problem of intra - segment inconsistency in prediction results and significantly improves the accuracy and reliability of surgical phase recognition.

Multi-objective constraints for path planning in screw fixation of scaphoid fractures

Purpose: Scaphoid fractures, a common type of clinical fracture, often require screw placement surgery to achieve optimal therapeutic outcomes. Path planning algorithms can avoid more risks and have vital potential for developing precise and automatic surgeries. despite the success of surgical path planning algorithms, automatic path planning for scaphoid fractures remains challenging owing to the complex bone structure and individual variations. Methods: Thus, we propose a Multi-objective constrained Path planning Algorithm (MPA) for fracture screw placement, which includes the identification of the center of the fracture surface. Further, three constraint conditions were introduced to eliminate infeasible paths, followed by adding three objectives to the remaining paths for more accurate planning. Finally, the Nondominated Sorting Genetic Algorithms (NSGA)-II algorithm was used to optimize the surgical paths. Results: We defined the vertical compression distance (VCd), a common observation index in clinics. The experiments show that the average VCd of the MPA paths is measured at 23.88 mm, outperforming the clinical planning paths by 21.71 mm. Ablation experiments demonstrated that all three objectives (distance, length, and angle) effectively optimized the path planning. Additionally, we also used finite element analysis to compare and analyze the MPA path and clinical path. The experimental results showed that the MPA path always outperformed the clinical path in terms of scaphoid strain and screw stress. Conclusion: This study presents a solution for the path planning of scaphoid fractures. Our future research will attempt to enhance the model's performance and extend its application to a broader range of fracture types. © 2024 Elsevier Ltd

X-ray angiographic images are regarded as the gold standard in the intra-operation of vascular diseases due to their high imaging quality and speed. However, due to many vascular- similar structures existing in the images, the accurate segmentation of the coronary artery is challenging. In this paper, we proposed a width-aware U-Net model by supervising the segmentation procedure using the label mask and centerline- derived width map. In our model, the centerlines of label masks are extracted and width maps are then computed. Together with the label mask, the width maps are then utilized to supervise the segmentation network. By comparing the proposed network with other state-of-the-art models, we can achieve superior results in discarding non-vascular artefacts and preserving the integrity of the whole vascular tree. © 2022 IEEE.

However, due to the projection process of X-ray imaging, much spatial information has been lost. To accurately provide efficient spinal morphological and structural information, reconstructing the 3-d structures of the spine from the 2-d X-ray images is essential. It is challenging for current reconstruction methods to preserve the edge information and local shapes of the asymmetrical vertebrae ***: In this study, we propose a new Edge-Aware Reconstruction network (EAR) to focus on the performance improvement of the edge information and vertebrae shapes. In our network, by using the auto-encoder architecture as the backbone, the edge attention module and frequency enhancement module are proposed to strengthen the perception of the edge reconstruction. Meanwhile, we also combine four loss terms, including reconstruction loss, edge loss, frequency loss and projection ***: The proposed method is evaluated using three publicly accessible datasets and compared with four state-of-the-art models. The proposed method is superior to other methods and achieves 25.32%, 15.32%, 86.44%, 80.13%, 23.7612 and 0.3014 with regard to MSE, MAE, dice, SSIM, PSNR andfrequency ***: due to the end-to-end and accurate reconstruction process, EAR can provide sufficient 3-d spatial information and precise preoperative surgical planning guidance. © 2024, The Authors. Self-expanding sinus stents are often used in functional endoscopic sinus surgery to treat inflamed sinuses. The PROPEL self-expanding sinus stent offers mechanical support to the sinus cavity to prevent restenosis. The stent is made of a bioabsorbable material (PLGA) that disappears after wound healing. However, complications such as foreign body sensation and severe stent migration/expulsion have been reported after implantation. Little is known about the contact characteristics of self-expanding sinus stents from when the stent is crimped into the insertion device through to deployment into the sinus cavity. This current study developed a test platform to analyze the biomechanical behavior of the stent during this process. Three common bioabsorbable materials, PLGA, PCL and Mg alloy, were evaluated to understand how the choice of material affects the biomechanical characteristics of self-expanding sinus stents. The results showed that the material can have a considerable influence on the contact characteristics during crimping and deployment. When crimped, the PLGA and Mg alloy stents showed much higher plastic strain and contact stress than the PCL stent. When deployed, the PCL stent had the largest contact area (4.3 mm2) and the lowest contact pressure (0.1 MPa) on the inner surface of the sinus canal. The results indicate that PCL could be a suitable choice for self-expanding sinus stents. This current study provides a method for observing the biomechanical characteristics of sinus stents during stent crimping and deployment. © 2023 Elsevier Ltd
