Individuals with type 1 diabetes mellitus must effectively manage glycemia to avoid acute and chronic complications related to aberrations of glucose levels. Because optimal diabetes management can be difficult to achieve and burdensome, research into a closed-loop insulin delivery system has been of interest for several decades. this paper provides an overview, from a controlsystems perspective, of the research and development effort of a particular algorithm-the external physiologic insulin delivery system. In particular the introduction of insulin feedback, based on beta-cell physiology, is covered in detail. A summary of human clinical trials is provided in the context of the evolution of this algorithm, and this paper outlines some of the research avenues that show particular promise. this paper compares various proposals for codifying categorical attributes in a heart disease database so that numerical clustering algorithms can be applied to them. An approach for the codification of categorical attributes based on polar coordinates is proposed. this is compared with other codifications and methods for clustering mixed databases found in the literature. Our proposal has many advantages: it is relatively easy to understand and apply;the increment in the length of the input matrix is not excessively large;and the committed error is under control. the proposed codification has been combined in this case withthe well-known k-means algorithm and has shown a very good performance in a heart disease database benchmark. In this paper we present the development of a modelling convention and a modelling environment, called PetDetSim, able to describe different PET detector block geometries. Using the developed simulation environment, the PET detector block designer can easily define different detector block configurations and test their optical behaviour regarding the characteristic features that determine the imaging quality of the device. In addition to the common optical behaviour of the detector block the developed modelling environment is also able to model gamma photon penetration. the PetDetSim environment is designed to use on a computer cluster and in a grid computing environment in order to reduce the computation time of the simulation. the validation of PetDetSim has been successfully done in the case of basic detector models, i.e. consisting of a single crystal needle. the model-based Karlsburg Diabetes Management System (KADIS (R)) has been developed as a patient-focused decision-support tool to provide evidence-based advice for physicians in their daily efforts to optimize metabolic control in diabetes care of their patients on an individualized basis. For this purpose, KADIS (R) was established in terms of a personalized, interactive in silico simulation procedure, implemented into a problem-related diabetes health care network and evaluated under different conditions by conducting open-label mono- and polycentric trials, and a case-control study, and last but not least, by application in routine diabetes outpatient care. the trial outcomes clearly show that the recommendations provided to the physicians by KADIS (R) lead to significant improvement of metabolic control. this model-based decision-support system provides an excellent tool to effectively guide physicians in personalized decision-making to achieve optimal metabolic control for their patients.
