Nuclear retinoid acid receptor beta in bronchial epithelium of smokers before and during chemoprevention

Background: Retinoids can reverse neoplastic lesions and prevent second primary tumors in the aerodigestive tract. These effects are thought to be mediated by nuclear retinoic acid receptors (RARs) and retinoid X receptors (RXRs), each receptor group including three subtypes (alpha, beta, and gamma), Previously, we found that RAR beta expression was suppressed in lung cancer. In this study, we investigated whether expression of RAR beta is modulated by chemopreventive intervention. Methods: Using in situ hybridization, we analyzed RAR beta messenger RNA (mRNA) expression in bronchial biopsy specimens from heavy smokers, at baseline and after 6 months of treatment with 13-cis-retinoic acid (13-cisRA) or placebo. Since we had previously detected RAR beta expression in 90% of bronchial specimens from nonsmokers, we considered loss of RAR beta mRNA expression in at least one of six biopsy specimens at baseline in this study to be aberrant. Results: RAR beta mRNA expression was aberrant in 30 (85.7%) of 35 subjects in the 13-cis-RA group and in 24 (72.7%) of 33 subjects in the placebo group. After 6 months of 13-cis-RA treatment, the number of subjects who were RAR beta positive in all six biopsy specimens increased from five of 35 to 13 of 35 (2.6-fold), so that the percentage of individuals with aberrant RAR beta expression decreased to 62.9% (22 of 35), which represents a statistically significant difference from baseline expression (two-sided P = .01). In the placebo group, no statistically significant difference in RAR beta expression was observed between baseline and 6 months, RAR beta expression was not related to current smoking status or reversal of squamous metaplasia. Conclusions: These results indicate that RAR beta is an independent marker of response to 13-cis-RA and may serve as an intermediate biomarker in chemoprevention trials of upper aerodigestive tract cancers.

Treatment outcome of acne vulgaris with oral isotretinoin in 89 patients

This is a retrospective study of 114 patients who received oral isotretinoin for acne vulgaris for a minimum of 6 weeks between January 1994 and March 1995. Relapse was defined as deterioration in acne sufficient to merit systemic therapy, either with antibiotics or with another course of isotretinoin. Patients were considered nonrelapsers after a minimum period of 1 year post-treatment with isotretinoin. Case notes were traced and the following data were retrieved: age, sex, duration of acne, site of acne, previous treatment with either antibiotics or isotretinoin, indications for treatment, total cumulative dose of oral isotretinoin, average daily dose of isotretinoin, response! relapse, time taken to relapse, and subsequent treatment. Those with either complete or partial (>80%) clearance were considered as responders because of the difficulty in categorizing them accurately due to the retrospective nature of the study. A telephone interview was conducted as far as possible with all patients who had an inadequate follow-up period to ascertain if they had relapsed, i,e, if they had been prescribed systemic therapy for further flares of acne, Analyses of laboratory abnormalities were performed only in patients who had serial alanine aminotransferase (ALT), total cholesterol, and triglyceride readings, Data were analyzed using the chi-squared test, Student's t-test, Mann-Whitney U-test, and Fisher's exact test. Of the 114 cases studied, 21 were excluded because of the following reasons: no weight was documented, 12;antibiotics were started immediately after completion of treatment making it impossible to document relapse, 2;patients were not seen at all after completing treatment and were uncontactable, 7, A further four cases with a break of 4 weeks or more during treatment were excluded as the accuracy of the calculated total cumulative dose of isotretinoin may be affected, given the half-life of 10-20 h.

Granuloma annulare following waxing induced pseudofolliculitis - resolution with isotretinoin

We report a case of localized granuloma annulare, successfully treated with isotretinoin, which occurred as a consequence of waxing-induced pseudofolliculitis, To our knowledge this is the first reported clearance of localized granuloma annulare with isotretinoin and the first reported case of granuloma annulare as a sequel to pseudofolliculitis.

Cancer chemoprevention: Present status, future expectations

Rather than attempting to treat cancel, after it has developed-and often metastasized-a more promising alternative is to direct therapy at the precancerous cell. For cancers linked to smoking, especially breast cancer and cancers of the head and neck, retinoids and tamoxifen have proven their ability to interrupt the carcinogenic process. However,widespread adoption wilt require further refinements.

Isotretinoin therapy for antibiotic-refractory acne in Apert's syndrome

Apert's syndrome is a congenital disorder characterized by the premature fusion of the cranial sutures and early epiphyseal closure. Severe pustular acne is a common feature. We report a case that failed to respond to conventional antibiotic treatment but resolved following isotretinoin therapy. The patient's permanent tracheostomy proved a managent challenge during isotretinoin treatment. The role of androgen-sensitvity is discussed in relation to the skin and bones.

Rapid response of IgA pemphigus of subcorneal pustular dermatosis type to treatment with isotretinoin

Diagnosing IgA pemphigus and distinguishing between its 2 subtypes, intraepidermal neutrophilic IgA dermatosis type and subcorneal pustular dermatosis type, is important because treatment of IgA pemphigus has to be different from treatment of other blistering autoimmune dermatoses. We present a patient with subcorneal pustular dermatosis type of IgA pemphigus who rapidly responded to systemic treatment with isotretinoin. Specific diagnosis was established by detecting IgA serum activity to desmocollin 1 by indirect immunofluorescence microscopy on unfixed COS7 cells transfected with desmocollin 1. No IgA or IgG serum reactivity was found to recombinant forms of desmogleins 1 and 3 by an antigen-specific enzyme-linked immunosorbent assay The disease was not effectively controlled by conventional therapeutic regimens. Systemic treatment with isotretinoin 20 mg daily led to complete clearance of skin lesions within 3 weeks. Assaying IgA serum reactivity to desmocollin 1, desmoglein 1, and desmoglein 3 as a valuable method for establishing the diagnosis and differentiating the 2 subtypes of IgA pemghigus. Isotretinoin was an effective drug in the treatment of subcorneal pustular dermatosis type of IgA pemphigus in this patient.

Acne fulminans is characterized by the sudden onset of a severe, ulcerative acne associated with systemic features. Response to traditional acne therapies is poor. We have recognized a subset of patients with acne of a severity comparable to that of acne fulminans but with the absence of systemic involvement: we suggest modification of the treatment regimes used in this group.

Concomitant administration of vitamin E does not change the side effects of isotretinoin as used in acne vulgaris: A randomized trial

Background: Isotretinoin treatment is frequently associated with reversible, dose-related side effects. Recent studies claimed that combining vitamin E with high-dose isotretinoin ameliorated isotretinoin-induced side effects. Objective: The purpose of this double-blind, randomized study was to determine the effects of a fixed dose of vitamin E on the side effects of isotretinoin for treatment-resistant acne vulgaris. Methods: One hundred forty subjects were randomly assigned to one of two treatment programs with isotretinoin (1 mg/kg) together with either vitamin E (800 IU/day) or a vitamin E placebo for 20 weeks. The incidence, severity, and duration of the side effects (eg, dry eyes, dry lips) were assessed. Results: A fixed 800 IU/day dose of vitamin E did not improve the incidence, severity, or duration of side effects associated with isotretinoin (1 mg/kg per day). Conclusion: Vitamin E did not significantly ameliorate retinoid side effects when combined with 1 mg/kg of isotretinoin in the treatment of acne.

目的观察和评价异维A酸胶丸联合克林霉素磷酸酯凝胶治疗中重度痤疮的疗效及安全性。方法门诊随机选择中重度痤疮患者136例,分治疗组和对照组各68例,治疗组口服异维A酸胶丸20mg/d,对照组口服维胺脂胶囊100mg/d,均局部外用克林霉素磷酸酯凝胶。两组均治疗8周,治疗前及治疗后2、4、6、8周观察皮疹改善程度,计算疗效指数,观察不良反应。结果经过8周观察,治疗组完成治疗65例,有效率96.9%,对照组完成治疗64例,有效率68.8%。治疗组有16例出现不良反应,对照组有8例出现不良反应,症状轻微,患者均可耐受。结论异维A酸胶丸联合克林霉素磷酸酯凝胶治疗中重度痤疮疗效高,不良反应轻微,可耐受,可作为临床有效治疗中重度痤疮联合方案之一。
