IT IS WITH GREAT HUMILITY and pride that I accept the distinct honor of presenting the 33rd Waiter J. Zeiter Lecture. I want to thank the American Academy of Physical Medicine and Rehabilitation (AAPM&R) Awards Committee and the Board of Governors for affording me this significant honor. It is very humbling to be recognized by one's peers and to realize that recognition indicates that one shares with Waiter Zeiter his attributes of statesmanship, scholarship, executive leadership, and warm friendship. In keeping with the tradition of the Zeiter Lecture, I will review the history of our field, and in doing so hope to persuade you that collaboration among rehabilitation medicine organizations and consumer groups is a logical extension of that history.(1).

The author reviews the history of advocacy for mentally ill individuals. Through organizations such as the National Mental Health Association and the National Alliance for the Mentally ill, collaboration among professionals, consumers, and concerned citizens is enhanced. The common causes and differences among organizations are discussed within the context of how psychiatrists can realize leadership strategies to further advance advocacy for mentally ill persons and for the profession.

This article is an attempt to create a general theoretical model or ‘scale’ for categorizing voluntary organizations chiefly concerned with social service provision. Consumer participation, which the author believes has positive and particular advantages in voluntary organizations, is taken as one element in the categorization. He discusses what participation may or should mean, and the reasons why it is so rarely found. Where it exists it tends to involve the participation of staff rather than ‘clients’, which may result in more discussion than action. The article also discusses how an organization's position on the ‘scale’ can change. Voluntary organizations which start out as small, highly-participating, self-help organizations tend to become more elitist and altruistic with success and growth. The author suggests that organizations with a higher degree of consumer participation may be more responsive to social change and its consequences, more flexible and even more cost-effective than more traditionally organized institutions and may also be better able to take advantage of the independence afforded only to voluntary organizations in an increasingly state-dominated society.

The organizational history of the visiting nurse association (VNA) is reviewed from the perspective of paradigm change. A paradigm is a behavioral and structural model that comes to characterize a field because of its compatibility with environmental conditions. The concept is applied to interpreting adaptation among the VNA's. Four distinct paradigms are identified which comprise the overlapping but sequential course of VNA adaptation to changing circumstances. These paradigms are "public health nursing," "community nursing service," "the home health agency," and the "home care enterprise." The emergence of each is examined with emphasis on the continuity and discontinuity of elements. Particular focus is given to the durability to date of the voluntary auspice and its future prospects in light of ongoing paradigmatic change is discussed.

Models of program development have primarily focused on the internal organizational processes needed to plan, implement, and evaluate new service programs. However creating an external demand for new programs by policy makers, administrative bureaucracies, public health officials, and funders is critical to establishing new programs. A series of deaths of seropositive youth and an absence of local service settings with staff trained to address the needs of youth living with HIV provided the impetus for Larkin Street Youth Center In particular;the agency had to overcome stigma associated with having both substance use and mental health disorders to establish service programs to recruit and mobilize staff within the agency and the local community and to establish a comprehensive housing program for symptomatic HIV-infected adolescents. This article examines how a residential assisted care facility for HIV-seropositive adolescents was established using organizational strategic planning processes, problem solving, and social marketing frameworks.

Private voluntary social welfare organizations (PVSOs) are operating under increasingly competitive conditions. "Strategic Planning" (SP) is a management tool designed to meet those conditions. In an exploratory study of 154 PVSOs the authors examined management's reasons for adopting strategic planning, the attention given to external analysis, and the degree of change and satisfaction achieved. PVSOs use SP because they are required/urged to do so by external agencies. External analysis received the least attention. One-half of the organizations achieved major change which was associated with high levels of satisfaction. Widespread stakeholder participation and the use of consensus strategies detracted from both major change and high satisfaction outcomes.
