Ergometric score, systems after myocardial infarction: Prognostic performance of the Duke Treadmill Score, Veterans Administration Medical Center Score, and of a novel score system, GISSI-2 Index, in a cohort of survivors of acute myocardial infarction

Background The aims of the study were to evaluate the performance of the Duke Treadmill Score (DTS) and. the Veteran Affairs Medical Center Score (VAMCS) in predicting 6-month death in GISSI-2 study survivors of acute myocardial infarction treated with thrombolytic agents, and to develop a simple predictive scoring system from the same database. Methods Patients of the GISSI-2 study (n = 6251) performed a maximal symptom-limited exercise test 1 month after myocardial infarction. We calculated for each patient the DTS and the VAMCS. Based on the coefficients of a multivariate analysis of our database, we developed a simple predictive scoring system and performed an internal validation. The prognostic value of each scoring system was assessed by multivariate analysis. Results six-month mortality rates in the subgroups of each scoring system were as follows: DTS: low risk 0.6%, moderate risk 1.8%, high risk 3.4% (P less than or equal to .0001);VAMCS: low risk 0.6%, moderate risk 1.9%, high risk 4.7% (P less than or equal to .0001);GISSI-2 Index: low risk 0.5%, moderate risk 1.9%, high risk 6.1% (P less than or equal to .0001). The results of multivariate analysis (relative risk [RR] and 95% Cl) were as follows: DTS: moderate risk 2.50 (1.47-12.59), high risk 5.13 (3.61-15.55);VAMCS: moderate risk 2.65 (1.53-4.59), high risk 5.97 (3.10-11.49);GISSI-2 Index: moderate risk 3.16 (1.81-5.52), high risk 8.65 (4.36-17.18). Conclusions The use of ergometric-derived prognostic score systems in a population of survivors of acute myocardial infarction treated with thrombolytic drugs distinguishes subgroups at different risks of death and allows an appropriate recourse to more costly procedures.

Muscle test comparisons of congruent and incongruent self-referential statements

This study investigated differences in values of manual muscle tests after exposure to congruent and incongruent semantic stimuli. Muscle testing with a computerized dynamometer was performed on the deltoid muscle group of 89 healthy college students after repetitions of congruent (true) and incongruent (false) self-referential statements. The order in which statements were repeated was controlled Ly a counterbalanced design. The combined data showed that approximately 17% more total force over a 59% longer period of rime could be endured when subjects repeated semantically congruent statements (p < .001). Order effects were nor significant. Or-rr-all, significant differences were found in muscle-rest responses between congruent and incongruent semantic stimuli.

Identifying submaximal muscular effort: Reliability of difference scores calculated from isometric and isokinetic measurements

The present investigation examined the reliability of a derived strength measurement and also how variability between test sessions affects the utility oh this variable for judging an individual's muscular effort. 31 young healthy men (M age = 25 yr.) completed three isometric and isokinetic concentric contractions of the knee extensors, using maximal and self-selected submaximal efforts, on each of two test yes sions. Difference scores between isometric and isokinetic measurements were calculated by subtracting the mean of the three isokinetic peak torques from the mean of the three isometric peak torques for each individual subject for maximal and submaximal efforts performed on both test sessions. For the group of subjects, difference scores were significantly greater during maximal (33 +/- 29 Nm) than submaximal (13 +/- 30 Nm) efforts, suggesting subjects could not maintain the same relationship between isometric and isokinetic muscular actions across the maximal and submaximal conditions. However, the test-retest reliability of the difference scores was only modest even when data were averaged over two test sessions (intraclass correlation coefficients were .82 for maximal, and .58 for submaximal). As a result, the range of scores within which an individual's true score might be expected to lie was large (+/- 25 Nm for maximal, and +/- 37 Nm for submaximal). Although derived strength parameters like difference scores may be effective in distinguishing submaximal from maximal efforts completed by groups of subjects, the test-retest reliability of the present scores suggests that their use in judging an individual's performance may be limited.

Relation of repetition effect and response programming in a speeded choice task

The present study tested the hypothesis that the repetition effect occurs during response programming. The choice reaction-time to initiate the second of two responses was examined when two consecutive responses were the same or different in their kinematics and force characteristics and repeated for two different stimuli. 12 subjects were required to react and produce the sequence of same or different force by squeezing a handle as quickly and accurately as possible after the first (auditory) and the second (visual) reaction signals. The response-stimulus interval. was set at 500 msec. The choice reaction-rime to initiate the second response was significantly shorter for the same-force condition than for the different-force and control conditions. This result indicates that the repetition effect originates in a speedup in response programming rather than response selection or perceptual identification. This finding was discussed in terms of bypassing a response-programming stage.

Reproducibility of performance and certainty of judgment in maximal vs feigned muscular effort

Detection of feigned muscular effort is a highly challenging topic in the medicolegal arena. Modem dynamometric methods have enabled a much higher accuracy in deciding whether the effort in question is indeed genuine or submaximal. Since in feigning the target, however, performance may fluctuate even within a single testing session, reproducibility cannot be assured. In actual claims, the statistical nature of certainty in proclaiming a feigned effort must be well understood Ly those involved in deciding the fate of a claim.

Intertester reliability of hand-held dynamometry: A concise summary of published research

Literature describing the use of hand-held dynamometry was examined to determine whether conclusions could be drawn regarding intertester reliability. 18 studies reporting intertester reliability coefficients were found and summarized. These suggest that high intertester reliability can be obtained, but that it is in jeopardy if tester strength is low relative to the forces being measured.

Relationships between impairments in strength of limb muscle actions following stroke

To examine the relationships between impairments in limb muscle strength soon after stroke, a secondary analysis of a data base of 48 patients with stroke was undertaken. Measurements of isometric muscle strength obtained bilaterally from eight muscle actions and recorded in the data base were retrieved for analysis. Most of the strength measures correlated significantly with one another. The measures of muscle strength showed high internal consistency in each limb, on each side, and over-all. Principal components analysis and cluster analysis indicated that the closest relationships were between muscle actions of the upper limb contralateral to the stroke (the ''paretic" side), limb muscle actions of the side ipsilateral to the stroke (the "nonparetic" side), and muscle actions of the lower limbs. These findings may reflect the organization of the central nervous system following stroke.

Measurement of upper and lower body strength and its relationship to underhand pitching speed

This study investigated the possible relationship for legs, arms, shoulders, and grip strength with underhand throwing speed. 40 female subjects had their legs, arms, and shoulder strength measured using a multipurpose exercise machine for assessing resistive force. Grip strength was measured using a handgrip dynamometer. Underhand throwing speed was measured with a radar gun. Regression analysis indi- cared a relationship (p less than or equal to .05) between strength of the arms and underhand pitching speed. No significant relationships were found for legs, shoulders, and grip strength, and underhand pitching speed.

This retrospective study summarizes the range of knee extension forces associated with the manual muscle lest grade of 5. The range of the forces (>560 Newtons) was large and encompassing of more than 86% of measurable forces. These findings help to explain the insensitivity of manual muscle testing and why it has such a profound ceiling effect.
