目的　探讨影响我国子宫颈癌高发区人乳头瘤病毒感染的因素 ,为人乳头瘤病毒感染和子宫颈癌的防治工作提供依据。方法　采用横断面调查法对山西省阳城县子宫颈癌筛查结果进行比较研究。将采集的子宫颈细胞应用第二代杂交捕获试验进行 HPV DNA检测 ,可检测 13种高危型 HPV脱氧核糖核酸 (DNA) ,以≥ 2 .0 pg/ ml作为阳性指标。资料应用 VFP软件进行录入和整理 ,用χ2 检验和非条件 L ogistic回归模型分析危险因素与子宫颈癌的关系。结果　 32 33名完成了调查表和 HPV检测的妇女符合本次研究的要求 ,各性生活持续年龄段 HPV感染率差别无显著性 (χ2 =6 .35 ,P=0 .5 0 )。单因素分析显示与 HPV感染相关的因素有婚外性行为、结婚次数 (>1/ 1)、子宫颈糜烂、息肉、尿路感染和阴道滴虫 ,OR分别为 1.6 9(1.38～ 2 .0 6 )、1.6 6 (1.19～ 2 .31)、1.4 1(1.0 8～ 1.84 )、1.2 4 (1.0 5～ 1.4 6 )、1.5 7(1.2 0～ 2 .0 7)、1.2 6 (1.0 4～ 1.5 2 )。 HPV感染的危险度比值比随性伴侣数增高呈增高趋势 ,与 1个性伴侣相比 ,2、3和 >3个的危险度比值比分别为 1.37(1.0 8～ 1.74 )、1.5 5 (1.0 3～ 2 .34)和 1.6 6 (1.19～2 .31)。多因素非条件 L ogistic回归分析结果表明 ,婚外性行为和尿路感染与

Prediction of high-grade cervical intraepithelial neoplasia in cytologically normal women by human papillomavirus testing

Human papillomavirus (HPV) testing has been suggested for primary screening of cervical cancer. Prediction of future high-grade cervical lesions is crucial for effectiveness and cost. We performed a case control study in a retrospective cohort of women with at least two cervical smears, all but the last one being negative, from the organized cervical screening programme in Florence, Italy. We searched for high-risk HPV in all previous, archival, smears from cases (new histologically confirmed cervical intraepithelial neoplasia (CIN) grade It or worse) and in one previous smear from each control (last smear cytologically normal, matched by age and interval (latency) from last smear). We applied polymerase chain reaction (PGR), and the b-globin gene was used as a DNA preservation marker. High-risk HPV was identified in 71/92 (77.17%) previous smears from 79 cases and 17/332 controls (5.12%). The adds ratio (OR) was 63.76 (95% CI 30.57-132.96). Among cases the proportion of HPV-positive smears declined slightly with increasing latency. Among cases, HPV was found in 81.24% (95% Cl 69.93-88.96%) of smears with latency < 4 years and in 67.80% (95% CI 47.72-82.93%) of those taken at longer intervals, up to 6 years. These findings suggest that testing far high-risk HPV allows predicting 80% of CINII/III 3 years before the cytological diagnosis and two thirds 6 years before. They also suggest that testing women negative for high-risk HPV at longer interval and strictly following-up women who are HPV positive could be an effective strategy for cervical cancer screening. Background: Human papillomavirus (HPV) infection has been strongly associated with cervical carcinoma and its cytologic precursors, squamous intraepithelial lesions (SIL). We investigated the risk of SIL prospectively following polymerase chain reaction (PCR)based DNA testing for a wide range of genital HPV types in a cohort of initially cytologically normal women, to clarify the role of HPV in the etiology of SIL. Methods: Starting in April 1989, 17 654 women who were receiving routine cytologic screening at Kaiser Permanente (Portland, OR) were followed for the development of incident SIL. During follow-up, 380 incident case patients and 1037 matched control subjects were eligible for this nested case-control study. Cervical lavages collected at enrollment and, later, at the time of case diagnosis (or the corresponding time for selection of central subjects) were tested for HPV DNA using a PCR-based method. The data were analyzed as contingency tables with two-sided P values or, for multivariable analyses, using odds ratios (ORs) with 95% confidence intervals (CLs). Results: In comparison with initially HPV-negative women, women who tested positive for HPV DNA at enrollment were 3.8 times (95% CI = 2.6-5.5) more likely to have low-grade SIL subsequently diagnosed for the first time during follow-up and 12.7 times more likely (95% CI = 6.2-25.9) to develop high-grade SIL. At the time of diagnosis, the cross-sectional association of HPV DNA and SIL was extremely strong (OR = 44.4 and 95% CI = 24.2-81.5 for low-grade SIL and OR = 67.1 and 95% CI = 19.3-233.7 for high-grade SIL). HPV16 was the virus type most predictive of SIL, even low-grade SIL. Conclusions: These findings are consistent with the hypothesis that HPV infection is the primary cause of cervical neoplasia, Furthermore, they support HPV vaccine research to prevent cervical cancer and efforts to develop HPV DNA diagnostic tests.

Human papillomavirus (HPV) is now recognized as the driving force behind the dysregulation of the cell cycle that underlies cervical malignant transformation. Although nearly all cervical cancers contain HPV genomes, the vast majority of HPVs are not oncogenic but merely induce benign lesions. Because progressive abnormalities take at least a decade to develop into invasive cancers, young women with low-grade lesions can often be managed with watchful waiting rather than ablative therapy.

Does SV40 infection contribute to the development of human cancers?

A large number of recent studies have reported the detection of simian virus 40 (SV40) nucleotide sequences in a number of unrelated human cancers which include paediatric and adult brain tumours, pleural mesotheliomas, bronchopulmonary carcinomas and osteosarcomas. Most of the data have been gathered by the use of PCR assays. These observations imply that SV40 is circulating in human communities by person-to-person transmission and that the virus is capable of wide dissemination in the infected individual. These claims must be regarded with skepticism for a variety of reasons. There are several inconsistencies in the reported data;for example, there is a wide variation (0-23%) in the frequency of SV40 sequences in normal tissues. The results of PCR assays have been difficult to confirm by less error-prone and independent assays and the relationship of the virus to the tumour cell is not characterised. The SV40 sequences reported from humans are essentially identical to those of wild-type SV40;it is difficult to conceive that a highly species-specific polyomavirus like SV40 would cross the species barrier, become a human infection, and acquire pathogenic potential for the new host without significant adaptive change in its genome. The available evidence, although not extensive, does not indicate that SV40 is circulating in human communities. It is premature to discuss or speculate on the potential role of SV40 in the development of human cancer until the presence of SV40 in cancers and in human communities is established unequivocally, the risk factors for SV40 infection are elucidated, and the effect of SV40 exposure on the development of specific cancers is examined in the context of other known risk factors for those cancers. In 1994, Chang and colleagues (1) used representational difference analysis to show that a unique DNA sequence was associated with Kaposi□s sarcoma in patients with the acquired immunodeficiency syndrome (AIDS). These sequences, which are of nonhuman origin, are closely homologous to minor capsid and tegument protein genes of the Gammaherpesvirinae (rhadinovirus subgroup), herpesvirus samirii, and Epstein-Barr virus. This observation was subsequently extended to patients with Kaposi□s sarcoma who did not have human immunodeficiency virus (HIV) infection (2). Since its initial discovery, the Kaposi□s sarcoma–associated herpesvirus/human herpesvirus-8 (KSHV/HHV-8) virus has been linked to two other lymphoproliferative disorders: multicentric Castleman□s disease (3,4) and a newly recognized disease, body-cavity–based lymphoma or primary effusion lymphoma (5–7). Human herpesvirus-8 infection of bone marrow dendritic cells in HIV-seronegative patients with multiple myeloma has also been reported (8,9), although the initial observations have not been replicated (10). Current epidemiologic evidence indicates that human herpesvirus-8 infection has little or no effect on common cancer types or sites, including multiple myeloma (11–13). A few reports, however, have identified the virus in diseases associated with vascular hyperplasia, including follicular B-cell lymphoma (14), benign and angioimmunoblastic lymphadenopathy (15), angiolymphoid hyperplasia with eosinophilia (16), and angiosarcoma (17).

Human papillomaviruses and vulvar vestibulitis

Objective: To assess the relationship between human papillomavirus (HPV) infection and vulvar vestibulitis syndrome. Methods: From November 1995 to December 1997, 135 women with vulvar vestibulitis were compared with 322 controls who had no evidence of vulvar vestibulitis. Human papillomavirus DNA was amplified by polymerase chain reaction and detected with liquid-capture molecular assay. Results: Human papillomavirus DNA was found in 29.6% of cases and in 23.9% of controls (relative risk [RR] 1.4;95% confidence interval [CI] .8, 2.2). The prevalence of HPV tended to decrease with increasing duration of pain among cases. Thus, prevalences were 37.5%, 29.6%, and 22.0% for pain durations of 3-6 months, 7-12 months, and 13-24 months, respectively (P =.14). Prevalence of HPV also tended to increase with pain intensity among cases, but that association was not statistically significant (P =.57). Prevalence percentages for women with low, moderate, or severe pain were 27.5%, 28.8%, and 34.4%, respectively. Prevalence of HPV was slightly higher in cases with the most severe pain (34.4%) than in controls (23.9%) (RR 1.8;95% CI .8, 4.0). In cases with the most pain in the shortest time (3-6 months), prevalence of HPV was double that of controls (50% versus 23.9%) (RR 3.5;95% CI 1.0, 12.7;P =.054). Conclusion: There was little support for the idea that HPV might be related to vulvar vestibulitis. (Obstet Gynecol 2000;95:683-7. High-risk mucosal human papillomaviruses encode an EG oncoprotein, which binds the cellular p53 tumor suppressor protein, thereby marking it for degradation through the ubiquitin-mediated pathway. A common p53 polymorphism at codon-72 of exon 4 results in translation to either arginine or proline. Recently reported: data suggested an increased susceptibility to E6/ubiquitin-mediated degradation of the Arg72-p53 isoform and an overrepresentation of the homozygous Arg72-p53 genotype in cervical cancer patients. We have analyzed this polymorphism in a larger series of patients with cervical cancer and in controls in the Czech Republic. We found no statistically significant differences between the codon-72 p53 genotypes of cervical cancer patients and the control women. Based on these results, it is unlikely that Arg72-p53 is associated with an increased risk for human papillomavirus-associated cervical tumor development in Czech women.

PURPOSE: To describe the clinical features of lacrimal gland inflammation associated with Epstein-Barr virus infection. METHODS: The clinical records, laboratory data, and radiographs of patients who had inflammation of one or both lacrimal glands that had begun less than il weeks previously were reviewed. RESULTS: Sixteen patients with dacryoadenitis were encountered between 1980 and 1996, a cumulative frequency of approximately one case per 10,000 new ophthalmic outpatients. Six individuals had serologic or other evidence of recent Epstein-Barr virus infection and were distinguished by the presence of regional lymphadenopathy, no purulent discharge, and a duration of symptoms of 6 weeks. CONCLUSION: Epstein-Barr virus is a probable cause of unilateral and bilateral dacryoadenitis in young adults. (C) 2000 by Elsevier Science Inc. All rights reserved.
