【目的】探讨清癜饮对变应性紫癜患者血清可溶性血管细胞黏附分子1（sVCAM—1）和白细胞三烯B4（LTB4）水平的影响。【方法】选取2013年3月至2015年8月在本院诊治的变应性紫癜患者86例，按随机数表组法分为两组，每组43例。对照组给予常规变应性紫癜治疗，观察组在对照组的基础上加用清癫饮治疗。比较两组患者有效率及治疗前后sVCAM-1、LTB4水平。【结果】观察组总有效率为97．7％（42／43），明显高于对照组的93．0％（40／43），其差异有统计学意义（χ2=4．371，P=0．036）；两组患者治疗后体内血清中sVCAM-1、LTB4水平较治疗前均有下降，但观察组下降显著，其差异均有统计学意义（P〈0．05）。【结论】清癫饮治疗变应性紫癜患者的疗效满意，能有效降低患者血清中的sVCAM—1与LTB4水平，值得临床推广应用。

背景:降钙素基因相关肽可减轻心肌缺血再灌注损伤。肾上腺髓质素与降钙素基因相关肽有一定的同源性,因此,一些研究推测肾上腺髓质素也可能对心肌有保护作用。目的:探讨肾上腺髓质素对缺血再灌注大鼠心肌组织血管细胞黏附分子1表达的抑制及其对心肌缺血的保护作用。设计:实验对象为SD大鼠心脏缺血再灌注模型,完全随机分组,随机设计。材料:本实验于2003-12/2004-05在咸宁学院医学院实验动物中心完成。实验选用健康雄性SD大鼠24只。方法:24只雄性SD大鼠心脏制成离体心脏缺血再灌注模型,随机分为对照组和肾上腺髓质素1-50(1×10 E-9),(1×10 E-8),(1×10 E-7)mol/L组。心脏缺血60min,对照组用氧合KHB液再灌注60min,其他3组分别用氧合KHB液加肾上腺髓质素1-50(1×10 E-9),(1×10 E-8),(1×10 E-7)mol/L再灌注15min,再用KHB液灌注45min。收集冠状动脉流出液,检测肌酸激酶同工酶含量。留取左室心尖部心肌进行总RNA提取,采用RT-PCR法测定血管细胞黏附分子1mRNA的表达。主要观察指标:对照组及不同浓度肾上腺髓质素1-50组心肌组织VCAM-1 mRNA的表达。结果:电泳后,各组在194bp处均可见一明显的扩增条带,此为甘油醛-3-磷酸脱氢酶mRNA扩增片段,各组间甘油醛-3-磷酸脱氢酶mRNA的表达基本一致。对照组、肾上腺髓质素1-50(1×10 E-9)mol/L组在334bP处可见亮度强、边界明显清楚的条带为血管细胞黏附分子1 mRNA扩增片段。肾上腺髓质素1-50(1×10 E-8)mol/L组血管细胞黏附分子1 mRNA扩增条带亮度明显减弱。肾上腺髓质素1-50(1×10 E-7)mol/L组仅为亮度微弱且模糊的扩增条带。肾上腺髓质素1-50(1×10 E-8),(1×10 E-7)mol/L组扩增的血管细胞黏附分子1mRNA与甘油醛-3-磷酸脱氢酶mRNA的灰度值比值(0.6±0.31,0.5±0.36)明显低于对照组(1.2±0.52)(P<0.05)。结论:心肌缺血再灌注时,肾上腺髓质素1-50可呈浓度依赖性地抑制心肌组织血管细胞黏附分子-1mRNA的表达。

Prediction of recurrent events by D-dimer and inflammatory markers in patients with normal cardiac troponin I (PREDICT) study

Background The independent predictive value of d-dimer and inflammatory markers for the risk of recurrent adverse events in patients with acute chest pain but normal levels of cardiac troponin I (cTnI) remains unclear. Methods We studied 391 patients admitted to the hospital in 1 year with acute ischemic-type chest pain. Creatine kinase-myocardial band isoenzyme (CK-MB) mass and cTnI levels were measured in initial and 12-hour samples. Soluble intercellular adhesion molecule (sICAM)-1, vascular cell adhesion molecule (sVCAM)-1, sP-selectin, sE-selectin, high sensitivity C-reactive protein (hsCRP), interleukin-6 (IL-6), fibrinogen, and d-dimer levels were measured in initial samples. A 1-year incidence of death, myocardial infarction (MI), revascularization, or readmission with chest pain was determined (with death/MI as the primary end point). Results Patients with normal levels of CK-MBmass and cTnI (195/391 [50%]) were at a lower risk than patients with elevated levels of CK-MBmass or cTnI, but still had an important incidence of events (77/195[39%]). Marker elevation was defined as >75th percentile (upper quartile). Elevated d-dimer levels (>580 ng/mL) was predictive of death/MI (odds ratio, 5.4;95% CI, 1.5-20.2;P = .005). Elevated sP-selectin levels (>152 ng/mL;odds ratio, 3.2;95% CI, 0.9-11.6;P = .06) trended to increased death/MI rates, with weaker trends for elevated levels of hsCRP (>7.1 mg/L), 11.6 (>10.7 pg/mL), and ST depression. Other markers, other electrocardiogram changes, or classic risk factors were, not predictive of death/MI. With a multivariate analysis, d-dimer and sP-selectin were found,to be of independent significance for death/MI after adjustment for inflammatory, hemostatic, and electrocardiogram markers and d-dimer after adjustment for classic risk factors. Conclusion Normal cTnI levels after acute chest pain does not confer absence of future risk. Concurrent assessment of d-dimer and inflammatory markers may improve risk stratification.

Elevated soluble P-selectin levels are associated with an increased risk of early adverse events in patients with presumed myocardial ischemia

Background Cell adhesion molecules (CAMs) play a pivotal role in the interactions between leukocytes, platelets, and vascular endothelium. Soluble CAMs (sCAMs) are shed from cell surfaces and reflect cellular activation. Elevated levels of sCAMs have been reported in the acute coronary syndromes. We hypothesized, therefore, that sCAMs might prove of prognostic value in patients with acute chest pain presumed to be the result of myocardial ischemia. Methods One hundred twenty-six consecutive patients with chest pain, thought clinically to represent myocardial ischemia, were studied prospectively. Soluble intercellular adhesion molecule-1 (sICAM-1), vascular cell adhesion molecule-1 (sVCAM-1), E-selectin (sE-selectin) and P-selectin (sP-selectin) levels were assayed at presentation, as were cardiac troponin 1 (cTnl) and creatine kinase-MBmass (CK-MBmass). The primary study end point was the occurrence of a serious cardiac event (SCE) during the index admission or the subsequent 3 months. Results sP-selectin and cTnl levels were significantly higher among patients who had an early SCE (P = .006 and P < .001, respectively). Both remained independently predictive (P < .001) in a multivariate regression equation. The other independent predictor was a history of vascular disease (P < .05). No other markers were significant predictors of early outcome. Conclusion Elevated sP-selectin levels, but not those of other sCAMs, are predictors of early adverse events in patients with chest pain presumed caused by myocardial ischemia. Their utility in predicting the outcome of individual patients is, however, limited.

Inverse association between carotid intima-media thickness and the antioxidant lycopene in atherosclerosis

Background Antioxidants may prevent atherosclerosis by interfering with endothelial activation, which involves the expression of endothelial adhesion molecules. The aim of this study was to explore the relationship between plasma levels of some lipid-soluble antioxidants (gamma-tocopherol, gamma-tocopherol, lycopene, beta-carotene, and ubiquinone), carotid maximum intimamedia thickness (IMTmax), an index of atherosclerotic extension/severity, and soluble adhesion molecules (vascular cell adhesion molecule-1 [VCAM-1], intercellular adhesion molecule-1 [ICAM-1], and E-selectin), which are taken as a reflection of vascular cell expression of adhesion molecules. Methods We studied 11 healthy control subjects, 11 patients with uncomplicated hypertension (UH), and 11 patients with essential hypertension plus peripheral vascular disease (PVD) who were matched for age, sex, smoking habit, and body mass index. Results Patients with PVD had elevated IMTmax (2.7 [1.1-3.1 ] mm, median [range]) compared with both patients with UH(1.2 [0.8-2.4] mm) and control subjects (1.0 [0.6-2] mm). In patients with PVD, soluble (s)VCAM-1 and sICAM-1 were also significantly higher than in the 2 other categories. Plasma levels of lycopene had a trend toward lower values in patients with PVD compared with other groups (P = .13). A statistically significant correlation was found between lycopene and IMTmax (r = 0.42, P = .014) at univariate analysis, which persisted at multivariate analysis (P < .05) and was independent of low-density lipoprotein cholesterol, creatinine clearance, and plasma insulin. Plasma lycopene did not significantly correlate with any of the soluble adhesion molecules tested. Conclusions We conclude that the inverse relationship of plasma lycopene with IMTmax is compatible with a protective role of this natural dietary antioxidant in atherosclerosis, although the mechanism of protection does not apparently involve a decrease in endothelial activation measured through solubl

Intravenous prostaglandin E₁ reduces soluble vascular cell adhesion molecule-1 in peripheral arterial obstructive disease

Objectives Elevated levels of soluble (s) vascular cell adhesion molecule-1 (VCAM-1) and intercellular adhesion molecule-1, pointing to activation of cells involved in vascular inflammation, have been previously reported in peripheral arterial obstructive disease (PAOD). We tested the hypothesis that intravenous prostaglandin E-1 (PGE(1)) treatment, which produces clinical benefits in this condition, might decrease such levels. Methods Ten subjects (age range 58 +/- 10 years, 6 male, 4 female) with characterized Fontaine stage IIa to IV PAOD (ankle/arm pressure index <0.96) were entered into a treatment protocol with twice daily intravenous infusions of PGE(1) (alprostadil) at 120 mug per day, repeated for 10 consecutive days. Preinfusion and postinfusion plasma samples were stored for blind enzyme immunoassays of soluble adhesion molecules and the fibrinolytic marker tissue plasminogen activator, type-1 plasminogen-activator inhibitor, and D-dimer. Results Estimates of severity of pain at rest, consumption of analgesics, magnitude of trophic lesions, remission to lower Fontaine stages, and favorable changes in the venoarteriolar reflex documented significant beneficial effects of the treatment. Significant (P < .01) pretreatment and posttreatment reductions of in all soluble markers explored were found. Particularly, sVCAM-1 exhibited a significant decrease after each infusion, which was sustained at the last day of treatment (from 854 +/- 214 ng/mL to 775 +/- 215 ng/mL across the first infusion, from 773 +/- 146 ng/mL to 680 +/- 110 ng/mL across the last infusion). Conclusion Thus a global decrease of vascular cell activation appears to occur as a result of PGE, administration and may contribute to the observed clinical benefits in PAOD.

Correlation between increased nitric oxide production and markers of endothelial activation in systemic sclerosis -: Findings with the soluble adhesion molecules E-selectin, intercellular adhesion molecule 1, and vascular cell adhesion molecule 1

Objective. To determine the relationship between vascular function and the inflammatory response in systemic sclerosis (SSc), and to investigate whether production of endothelial-derived nitric oxide (NO) is disturbed in this disease. Methods. We measured plasma nitrate, urinary excretion of both nitrate and cGMP, and soluble adhesion molecules of endothelial origin in patients with SSc and in age- and sex-matched controls and compared these levels between groups. Additionally, we performed correlation analysis to determine how these variables were related to one another, Plasma nitrate and 24-hour-urinary excretion of nitrate in patients and controls were measured after a 72-hour nitrate-free-diet, using a gas chromatography/mass spectrometric method. Soluble adhesion molecules intercellular adhesion molecule 1 (sICAM-1), vascular cell adhesion molecule 1 (sVCAM-1), and E-selectin and cytokines were measured by enzyme-linked immunosorbent assay. The expression of E-selectin was further investigated in skin biopsy specimens by immunoperoxidase staining, and the presence of inducible NO synthase by immunoblotting. Results. Plasma nitrate and 24-hour-urinary-excretion of cGMP were significantly elevated in patients compared with controls, while 24-hour-urinary-excretion of nitrate tended to be elevated in SSc patients. Levels of sICAM-1, sVCAM-1, and sE-selectin were significantly elevated in the patients. Levels of plasma nitrate in the patients correlated significantly with levels of sVCAM-1 (P = 0.020) and sE-selectin (P = 0.018) and approached a significant correlation with sICAM-1 (P = 0.055), suggesting that activated endothelial cells may produce plasma nitrate. Conclusion. NO synthesis is elevated in SSc patients, and the activated endothelial cell is a likely site of its production.

OBJECTIVES The purpose of this study was to evaluate whether soluble vascular cell adhesion molecule-1 (sVCAM-1) is a marker for increased cardiovascular risk. BACKGROUND Soluble forms of cellular adhesion molecules (CAMs) may be useful markers of endothelial activation and local or systemic inflammation. Recent studies indicate that plasma concentration of soluble intercellular adhesion molecule-1 (sICAM-1) is elevated many years before a first myocardial infarction (MI) occurs. However, only a few prospective studies have evaluated whether sVCAM-1 is also a marker for increased cardiovascular risk. METHODS Baseline plasma samples were obtained prospectively from 14,916 healthy participants in the Physicians' Health Study. In a nested, case-control study design, the plasma concentration of sVCAM-1 was measured in 474 men with confirmed MI during the nine-year follow-up period, and in an equal number of control subjects who remained free of reported cardiovascular disease and who were matched for age, smoking status and length of follow-up. RESULTS No significant difference in the median baseline sVCAM-1 concentration was found between case and control subjects (638 vs. 634 ng/ml;p = NS). Cardiovascular risk was similar between patients with sVCAM-1 levels in the highest quartile and those in the lowest quartile, in both crude (relative risk [RR] 1.28, 95% confidence interval [CI] 0.85 to 1.92) and adjusted (RR 1.17, 95%, CI 0.71 to 1.91) matched-pairs analyses. CONCLUSIONS In contrast to previous data on sICAM-1, we found no evidence of an association between sVCAM-1 levels and the risk of future MI in a large cohort of apparently healthy men. These data suggest important pathophysiologic differences between sVCAM-1 and sICAM-1 in the genesis of atherothrombosis. (C) 2000 by the American College of Cardiology.

Evidence of prolonged inflammation in unstable angina and non-Q wave myocardial infarction

OBJECTIVES This study was designed to document the inflammatory response up to one year after acute presentation with unstable angina (UA) and non-Q wave infarction (NQMI) as reflected by the expression of soluble cell adhesion molecules (CAMs). BACKGROUND Coronary plaque inflammation is a key component in the pathogenesis of acute coronary syndromes. Cell adhesion molecules are critical mediators of the inflammatory process. Soluble forms of these molecules are detectable in serum and are elevated acutely in patients with UA and NQMI. METHODS Patients presenting with UA and NQMI had serum samples taken at presentation and then after three, six and 12 months. A control group of similar age and gender distribution was used for comparison. Levels of soluble inter-cellular adhesion molecule-1, vascular cell adhesion molecule-1, endothelial-selectin and platelet-selectin were measured using an ELISA technique. RESULTS We studied 91 patients (M/F = 73/18, mean age 62 +/- 11 years, 56 UA and 35 NQMI) and 24 controls (M/F = 18/6, mean age 56 +/- 12 years). Levels of all four soluble CAMs were significantly elevated in both UA and NQMI patients at presentation, three and six months in comparison with controls. Levels in UA and NQMI groups fell between six and 12 months after initial presentation. CONCLUSIONS The results suggest that the inflammatory stimulus triggering expression of CAMs is sustained for up to six months after presentation with either UA or NQMI and then returns toward control values over the following six months. a Am Coil Cardiol 2000;36:1210-6) (C) 2000 by the American College of Cardiology.
