目的：探讨非清髓性异基因造血干细胞移植（ NST）治疗T细胞淋巴瘤的安全性和疗效。方法采用NST治疗T细胞淋巴瘤3例，均接受人类白细胞抗原全相合的同胞供给的异基因造血干细胞移植。预处理方案：马利兰＋氟达拉滨＋环磷酰胺，移植物抗宿主病（ GVHD）预防：环孢素联合短疗程甲氨蝶呤。结果3例患者造血均获快速重建，无急性GVHD发生，2例患者出现轻微的慢性GVHD，经泼尼松治疗后好转。3例患者移植后随访时间分别为：22个月、16个月、11个月。1例患者于移植后12个月复发，予供者淋巴细胞输注后再次完全缓解，其余2例患者仍无病生存。结论 NST治疗T细胞淋巴瘤安全有效，可为治愈T细胞淋巴瘤提供新的手段。

HEMATOPOIETIC ENGRAFTMENT FROM A MINIMAL NUMBER OF APHERESIS PROCEDURES AFTER MOBILIZATION OF PERIPHERAL-BLOOD STEM-CELLS WITH CHEMOTHERAPY AND RHG-CSF

In a cohort of 13 patients, peripheral blood stem cells (PBSC) were harvested by apheresis after mobilization with chemotherapy and rhG-CSF. Nine patients who had excellent mobilization were transplanted with PBSC concentrates from a minimal number of apheresis procedures (mean of 1.5, range = 1-3). During collection, the number of circulating progenitors was on average 50 times higher than those observed at the steady state in the peripheral blood of healthy unstimulated individuals. The mean number of CFU-GM/kg reinfused per patient was 28.1 x 10(4) (range = 18.0-50 x 10(4)). The use of rhG-CSF, at either 1 or 5 mu g/kg/day, resulted in a significantly greater yield of CFU-GM per mononuclear cells than that observed previously in a comparable group of patients receiving chemotherapy alone. Prompt and durable engraftment occurred after myeloablative chemotherapy. The average duration of absolute neutropenia was 9 days. Transfusion requirements were low with an average of four packed red cell units and two platelet transfusions per patient. The shortest follow-up is 5 months and the longest is 20+ months. The convenience of this new approach to support myeloablative therapy offers new possibilities for the administration of a higher dose-intensity of chemotherapeutic agents. A limited number of apheresis procedures timely harvested will improve the cost effectiveness of transplant programs.

An in vivo competitive repopulation assay for various sources of human hematopoietic stem cells

The marrow repopulating potential (MRP) of different sources of human hematopoietic stem cells (HSCs) was directly compared using an in vivo assay in which severe combined immunodeficient disease (SCID) mice were implanted with human fetal bones. HSCs from 2 human lymphocyte antigen (HLA)-mismatched donors were injected individually or simultaneously into the fetal bones of a 3rd distinct HLA type and donor and recipient myeloid and lymphoid cells were identified after 8 to 10 weeks. The study compared the MRP of umbilical cord blood (CB) and adult bone marrow (ABM) CD34(+) cells as well as grafts of each type expanded ex vivo. Equal numbers of CB and ABM CD34(+) cells injected individually demonstrated similar abilities to establish multilineage hematopoiesis. However, when CB and ABM cells were transplanted simultaneously, the engraftment of CB cells was markedly superior to ABM. CB and ABM CD34(+) cells were expanded ex vivo using either a porcine microvascular endothelial cell (PMVEC)based coculture system or a stroma-free expansion system. Primary CB CD34(+) cells or CD34(+) cells expanded in either culture system demonstrated a similar ability to engraft. However, the MRP of expanded grafts simultaneously injected with primary CB cells was uniformly inferior to primary CB cells. CD34(+) cell grafts expanded in the stroma-free system, furthermore, outcompeted CD34(+) cells expanded using the PMVEC coculture system. The triple HLA-mismatched SCID-hu model represents a novel in vivo stem cell assay system that permits the direct demonstration of the functional consequences of ex vivo HSC expansion and ontogeny-related differences in HSCs. Bone marrow: Target for gene transfer

Marrow cells are currently the most widely utilized target in human trials of gene therapies, and hematopoietic stem cells may be the best target of all. Able both to self-replicate and to engender blood cells in all lineages, they could be a lifelong source of therapeutic genes for disorders in any lineage. Hematopoietic stem cells may also portend the therapeutic use of even more versatile embryonic stem cells.

Ex vivo manipulations of hematopoietic stem and progenitor cells are increasingly used in the context of autologous and allogeneic stem-cell transplantation. These manipulations include the positive selection of CD34⁺ cells for tumor-cell reduction and/or T-cell depletion, the ex vivo expansion of hematopoietic progenitor and stem cells under appropriate cytokine-stimulated culture conditions, and the ex vivo generation of myeloid or megakaryocytic postprogenitor cells and immune effector cells. This article summarizes both the preclinical data on the ex vivo expansion of hematopoietic progenitor and stem cells from purified CD34⁺ cells and the initial clinical studies with ex vivo-expanded stem and progenitor cells for hematopoietic support after high-dose chemotherapy.

Adoptive allogeneic immunotherapy - history and future perspectives

For more than 30 yrs allogeneic hematopoietic stem cell transplantations have been successfully performed in patients with hematologic malignancies and bone marrow aplasia. Over the years the field of transplantation has changed dramatically. More and more unrelated donors became available, regimens for haploidentical transplantations were introduced and G-CSF mobilized peripheral blood stem cells and fetal cells from umbilical cord became available as alternate sources of hematopoietic stem cells. However, especially the introduction of donor lymphocyte infusions (DLI) for the successful treatment of leukemic relapses after allogeneic stem cell transplantations improved our understanding of transplantation immunology and opened amazing perspectives in allogeneic transplantation. It was long believed, that myeloablative therapy with high-dose chemotherapy and total body irradiation (TBI) are the sole antileukemic principles in allogeneic transplantations. But by now it became clear, that donor lymphocytes exert a very potent antileukemic effect, now referred as the graft-versus-leukemia (GVL) or graft-versus-malignancy (GVM) reaction. The efficacy of DLI in controlling leukemic relapses suggests that myeloablative therapy is not essential for longterm disease control. By exploiting the GVL or GVM reaction more intensively the role of chemotherapy and TBI is changing to immunosuppression. Sufficient immunosuppression to allow grafting, however, can be achieved with much lower doses as those which have been used in conventional transplants. Therefore allogeneic transplants have become also available for the elderly or for patients with concurrent medical conditions, which would have excluded them from conventional transplants. Moreover, this allogeneic transplantation strategy with reduced intensity conditioning is now also under investigation in patients with susceptible solid tumors and autoimmune diseases. Limited engraftment capacity of bone marrow-derived mesenchymal cells following T-cell-depleted hematopoietic stem cell transplantation

The engraftment capacity of bone marrow-derived mesenchymal cells was investigated in 41 patients who had received a sex-mismatched, T-cell-depleted allograft from human leukocyte antigen (HLA) matched or -mismatched family donors. Polymerase chain reaction (PCR) analysis of the human androgen receptor (HUMARA) or the amelogenin genes was used to detect donor-derived mesenchymal cells. Only 14 marrow samples (34%) from 41 consenting patients generated a marrow stromal layer adequate for PCR analysis. Monocyte-macrophage contamination of marrow stromal layers was reduced below the levels of sensitivity of HUMARA and amelogenin assays (5% and 3%, respectively) by repeated trypsinizations and treatment with the leucyl-leucine (leu-leu) methyl ester, Patients who received allografts from 12 female donors were analyzed by means of the HUMARA assay, and in 5 of 12 cases a partial female origin of stromal cells was demonstrated. Two patients who received allografts from male donors were analyzed by amplifying the amelogenin gene, and in both cases a partial male origin of stromal cells was shown. Fluorescent in situ hybridization analysis using a Y probe confirmed the results of PCR analysis and demonstrated in 2 cases the existence of a mixed chimerism at the stromal cell level. There was no statistical difference detected between the dose of fibroblast progenitors (colony-forming unit-F [CFU-F]) infused to patients with donor- or host-derived stromal cells (1.18 +/- 0.13 x 10(4)/kg vs 1.19 +/- 0.19 x 10(4)/kg;P greater than or equal to .97). In conclusion, marrow stromal progenitors reinfused in patients receiving a T-cell-depleted allograft have a limited capacity of reconstituting marrow mesenchymal cells. Hematopoietic stem-cell transplantation from an unrelated donor

When an HLA-matched sibling is not available and nontransplant approaches have been ineffective, transplantation of hematopoietic stem cells from an unrelated donor can effect a cure in some patients with chronic myelogenous leukemia Even when successful, however, transplantation is a rigorous and prolonged process whose complications include infection and graft-versus-host disease. Improvements in hematopoietic stem-cell transplantation as well as innovative nontransplant approaches may improve the outcome in these patients.
