WE-E-WAB-01: Solutions for Interconnectivity Issues in Radiation Oncology: Integrating the Healthcare Enterprise-Radiation Oncology (IHE-RO)

Integrating the Healthcare Enterprise in Radiation Oncology (IHE-RO) is an ASTRO sponsored initiative, composed of vendors, physicists and clinicians dedicated to improving equipment and software integration issues related to radiation treatment. The objective of this initiative is to improve the way computer based systems in radiation oncology share information using well-defined data exchange standards (DICOM / HL7). The Radiotherapy process requires integration of software and hardware from a variety of vendors using a multitude of systems. For this integration to be done successfully a series of data transfers must occur with 100% fidelity and reliability. Here there can be a significant problem of lack of interconnectivity and interoperability of electronic infrastructure. The follow-up care of radiation oncology patients extends over time and multiple care facilities. Therefore, it is critical that all patients' healthcare data be readily available on demand to individuals and their changing healthcare providers. The IHE-RO initiative has solved several interconnectivity and interoperability issues amongst Treatment Planning Systems, Treatment Management Systems and Treatment Delivery Systems. However, it is not easy for end users to discern the lack of interconnectivity amongst RT devices available in the market place. IHE-RO testing events and test tools have enabled vendors to successfully test and demonstrate interoperability between their products. These solutions will improve patient safety in concordance to the ASTRO ‘Target Safely’ initiative in addition to improving workflow efficiency in the *** 2004, ASTRO formed a multi-society, multi-national, and multi-specialty IHE-RO task force to address issues of interoperability and information sharing between various healthcare systems in radiation oncology and is working to resolve them in a systematic way using established industry standards. TH-C-137-01: A Graphical Software Tool for TrueBeam Developer Mode

Purpose: To introduce a novel graphical software tool to assist scientists and engineers in building new applications in TrueBeam Developer's Mode. Methods: As an option, TrueBeam users are provided with access to the full set of capabilities within the XML-based Developer's Mode. It is very laborious to compile a XML file, however, particularly when IMRT beams are included along with KV and MV imaging triggers. In this study, a choreographing software tool, TrueBeam TeachMod, was developed based on DICOMan-TX, a DICOM utility tool and a research platform for radiation therapy and radiology, to facilitate exploring potential technologies that can ultimately be applied to clinics. A DICOM RT plan from a treatment planning system is translated into a XML file which is further complemented with imaging triggers and axis changes. The tool allows the user to interactively edit any axis at any beam control point. It also includes predefined treatment and KV/MV imaging procedures that can be frequently used to build scientific applications. Results: IMRT plans from either static or dynamic were transferred via DICOM connection to DICOManTX where True Beam Teach Mod produced the coordinate beam components. Trajectories of all axes were graphically rendered and edited as needed. The process requires less than five minutes to generate sophisticated XML instructions containing tens of thousands lines. A series of test application was successfully implemented on the True Beam unit. Conclusion: A XML composing tool is highly desirable for users who want to extensively explore the potential of the True Beam platform. It not only provides the users with a tool that could expedite their research and development but also opportunities for them to conduct unconventional techniques that are prohibitively time consuming without it.

Purpose: To explore a method of using a computer system for multi-media cases conference in clinical radiation oncology, in order to replace the traditional use of film images and paper. Methods: In a conference room without any treatment planning system(TPS), on a dedicated networked computer, we use Remote Desktop feature of Windows XP to remote access an Varian Eclipse TPS and the radiation therapy information management system (RTIMS), which is an in-house developed system based on the flow of radiation therapy. Then in this room, we can online read the information of CT images, target volumes, treatment plans, electronic medical records and etc, and project it on the screen. Results: There has been half a year since we successfully set up a radiation therapy case conference multi-media system(RTCCMMS) in the department. With this method, we can share the TPS in another room, and do not need buy another TPS. It is convenient and effective that the oncologists and physicists conduct a collective case discussion. Conclusion: This method required equipments are simple, and it is a safe and reliable technology, greatly improving the clinical efficiency and quality of medical care. Accuracy Considerations in Modern Radiation Oncology: An Update

The most recent reviews of accuracy requirements in radiation oncology were published in the 1990s, primarily in an era that was transitioning from 2-D to 3-D conformal radiation therapy (CRT). Since then, the technology associated with radiation oncology has changed dramatically. The combination of various forms of imaging for radiation therapy planning, treatment planning software, dose delivery technology including 4-D considerations as well as in-room daily image guidance has resulted in new perspectives on accuracy considerations. The underlying hypothesis for the use of these advanced technologies is that loco-regional control of cancer remains a significant barrier to cancer cure for many common cancers and that better dose distributions will translate into better outcomes. However, further clinical gain using these new technologies may be limited by single or compounded uncertainties associated with the entire treatment process. Thus, it is important to understand what factors should be considered in determining accuracy requirements as well as the realistic expectations of uncertainties that exist within the total treatment process. The need for accuracy is based on clinical requirements such as the steepness of dose-response curves, inherent heterogeneity in patient response to treatment, and the level of accuracy that is practically achievable. Statements on accuracy are dependent on the technology used and the reality of what is practically achievable and necessary. This review highlights some of the major differences between accuracy requirements as determined in the 2-D RT and 3-D CRT era versus the modern era of intensity modulated, image-guided, 4-D radiation therapy.
