卵圆孔未闭(PFO)伴双房间隔(Double IAS)是一种罕见的心脏畸形,可能导致多种症状,但文献中报道的病例数量有限。然而,在这些病例中,仅凭传统经胸超声心动图成像往往难以作出正确诊断,有时甚至可能导致手术失败。因此,准确识别房间隔的结构异常至关重要。本报告描述了一例50岁女性患者的病例管理过程,该患者因慢性偏头痛就诊。Patent foramen ovale (PFO) with a double interatrial septum (Double IAS) is a rare cardiac abnormality that may result in various symptoms, with only a few cases reported in the literature. However, in those cases, it’s hard to diagnose correctly using only traditional transthoracic echocardiogram imaging, even though it may sometimes result in surgery failure. It is very important to correctly identify the structural abnormality of the atrial septum. This report describes the case management of a 50-year-old female who presented with chronic migraine.
目的:统计分析药品说明书中男性生育信息标注情况,为有生育计划男性的临床用药提供参考。方法:收集《国家基本药品目录(2023)》《全国药品集中采购中选结果表(2022)》以及重庆医科大学附属第三医院在院使用的药品说明书,采用逐一阅读的方式,对男性生育力标注信息进行提取、统计和分析。以PubMed、Web of Science、中国知网(CNKI)、万方数据库、维普中文期刊数据库(VIP)为数据源,检索有关药物对男性生育力影响的研究,对有文献报道了生殖毒性而未在说明书中载明的药品进行整理归纳。结果:共收集药品说明书2823份,其中西药说明书1644份,标注了男性生育力相关信息的占476份;中成药说明书1179份,仅7份标注了男性生育力相关内容,总标注率为17.11%。在西药说明书中,标注率最高的药物分类为抗肿瘤药(60.45%);进口药的标注率为48.01%,其中标注在“生育力/不孕”项下的占48.07%;国产药品说明书标注率为23.28%,其中标注在“生育力/不孕”项下的占29.83%。在所有调研说明书中,30个药品有文献报道生育毒性而未在说明书中阐明。结论:说明书中有关男性生育力信息总体标注率不高,绝大多数缺乏人类数据和用药指导,需要更多的循证医学证据来完善药品说明书,为医务人员、患者提供可靠的信息。

目的评价星状神经节阻滞(SGB)对全身麻醉手术患者术后睡眠质量的影响。方法计算机检索PubMed、Cochrane Library、Embase、Web of Science、中国知网、万方、维普中关于SGB对术后睡眠质量影响的随机对照试验(RCT),各库检索期限为建库至2023年7月21日。根据Cochrane5.1.0偏倚风险评估工具对筛选所得文献进行评估,之后采用RevMan5.4软件进行meta分析。结果共筛选纳入8篇文献。meta分析结果显示,SGB组术后首夜睡眠障碍发生率低于对照组(OR=0.26,95%CI:0.15~0.45,P<0.001),SGB组术后总睡眠时间高于对照组(MD=69.06,95%CI:53.69~84.44,P<0.001),SGB组睡眠效率高于对照组(MD=13.59,95%CI:10.70~16.49,P<0.001);SGB组术后阿森斯失眠量表(AIS)评分(MD=-1.73,95%CI:-2.43~-1.03,P<0.001)及匹兹堡睡眠质量指数(PSQI)评分(MD=-3.26,95%CI:-5.40~-1.12,P<0.003)低于对照组。结论SGB能降低术后首夜睡眠障碍发生率,延长术后总睡眠时间,提高术后睡眠效率,降低术后睡眠质量评分,改善术后睡眠质量,是一种效果好且安全性高的非药理学方法,临床应用前景好,值得推广。
脂代谢是机体重要代谢之一,肝脏作为脂质合成、储存以及代谢的中心器官,二者相互影响。肿瘤细胞所处的局部环境称为肿瘤微环境,为适应肿瘤微环境而发生的代谢改变称为代谢重编。目前脂代谢与肝癌之间的相关性尚不完全清楚,肝癌脂代谢重编的具体过程亦尚未完全揭示。本文介绍了肝癌发生、发展各阶段脂代谢特点,总结了肝癌脂代谢重编过程的最新发现,旨在探究脂代谢在肝癌发生、发展以及预后中的作用,为肝癌防治提供新的思路与依据。Lipid metabolism is one of the important metabolic processes in the body, and the liver is the central organ for lipid synthesis, storage, and metabolism, with a mutual influence between them. The local environment in which the tumour cells are located is called the tumor microenvironment, and the metabolic changes that occur to adapt to the tumor microenvironment are called metabolic reprogramming. At present, the relationship between lipid metabolism and liver cancer is not completely clear, and the specific process of lipid metabolic reprogramming in liver cancer has not yet been fully elucidated. This review introduces the lipid metabolic characteristics of liver cancer at each stage of its occurrence and development, summarizes the latest findings on the lipid metabolic reprogramming process in liver cancer, and aims to explore the role of lipid metabolism in the occurrence, development, and prognosis of liver cancer, providing new ideas and evidence for the prevention and treatment of liver cancer.

This article will review current ultrasound techniques used to assess left ventricular systolic and diastolic function in cirrhosis, and describe new imaging modalities with potential clinical applicability in the near future.
