Perioperative neurocognitive disorder(PND) is a significant neurological complication in aging perioperative patients that impacts postoperative cognition. PND is currently diagnosed through cognitive function testing, which is limited by its subjectivity and time requirements. Thus, the identification of biomarkers to assess PND onset is a priority to identify at-risk individuals and enable interventions and treatments to patient outcomes. This article synthesizes expert perspectives on brain aging and PND, presents the latest clinical evidence on PND biomarkers(imaging, electroencephalography, and molecular biomarkers), and delves into the relationship between PND and other age-related cognitive disorders. Thorough review of PND research identified several biomarkers with high sensitivity and specificity,offering a solid scientific foundation to predict and diagnose PND. These biomarkers not only enhance diagnostic accuracy for clinicians but also provide opportunities for earlier intervention and more effective treatment, potentially enhancing patient outcomes and quality of life.

Hearing loss often arises from impairments in multiple genes, complicating therapeutic development. MicroRNAs, as master regulators, offer promising targets for complex diseases. We explored miR-96’s roles in hair cell (HC) function and noise-induced hearing loss (NIHL), finding that miR-96^−/−, not miR-96^+/−, mice exhibited progressive hearing loss due to gene regulatory network dysregulation from miR-96 loss in HCs not spiral ganglion neurons (SGNs). Viral-mediated delivery of miR-96 into the inner ear partially rescued hearing of miR-96^−/− mice. Tamoxifen-induced depletion of miR-96 in adult UBC^CreERT2/+;miR-96^fl/fl mice led to hearing loss, with Bach2, Gabra2, Gabra4, and Grk1 upregulation and Tnn, Col11a1, Gjb3, and Hnf4a downregulation. Furthermore, noise trauma reduced miR-96, altering Bach2, Bcl2l1, Slc26a9, Gabrb1, Grk1, Nos2, and Cyp1a1 expression, whereas miR-96 overexpression protected hearing against noise by reversing the expression of Bach2, Bcl2l1, and Cyp1a1. Our findings underscore miR-96’s essential role in adult hearing maintenance and NIHL prevention, presenting it as a promising therapeutic target. © 2025 The Author(s)

Topologically protected spin textures, such as skyrmions and antiskyrmions, have garnered considerable attention in the past decade. Although theoretical models have proposed various mechanisms for stabilizing antiskyrmions, their experimental realization has predominantly been limited to systems exhibiting anisotropic Dzyaloshinskii-Moriya interactions and specific symmetry classes, such as D2d or S4. Alternatively, the stabilization of antiskyrmions can be achieved through the manipulation of the interplay between long-range dipolar interactions and exchange interactions. In this paper, we report the observation of dipolar-stabilized skyrmions and antiskyrmions in the van der Waals magnet Fe3GaTe2 (FGT) across a broad temperature range (100–370 K) without the need of an external magnetic field. By manipulating thermal agitation to modulate the strength of perpendicular magnetic anisotropy, we were able to precisely control the formation of skyrmions and antiskyrmions, as observed using Lorentz transmission electron microscopy. We further demonstrate the presence of multitopological spin textures in FGT and their reversible transformations, which were controlled through a history-dependent magnetic field sweep process. These phenomena were characterized using magnetic force microscopy. In this paper, we provide experimental evidence for the existence of topological quasiparticles in two-dimensional systems.

Schizophrenia-like phenotypes and long-term synaptic plasticity impairment in GluN2A-transgenic mice

While N-methyl-D-aspartate receptor (NMDAR) hypofunction has been suggested as a hallmark of schizophrenia, the role of subunit-specific dysregulation such as GluN2A overexpression remains poorly understood. The present study comprehensively investigated the impact of GluN2A overexpression on behavioral phenotypes, cognitive functions, and synaptic plasticity in transgenic mice with forebrain-specific overexpression of the GluN2A subunit (GluN2A-TG). Behavioral assessments revealed schizophrenia-like phenotypes, including prolonged stereotypic movement duration, impaired sensorimotor gating, reduced social interaction, and diminished nest-building activity in GluN2A-TG mice. Consistently, GluN2A-TG mice exhibited not only deficits in spatial working memory and olfactory working memory but also impaired associative learning. In addition, both long-term potentiation and long-term depression were significantly attenuated in the prefrontal cortex (PFC) of GluN2A-TG mice. Furthermore, electrophysiological analysis of NMDAR-mediated excitatory postsynaptic currents in PFC neurons revealed altered kinetics characterized by a faster decay time and significantly increased amplitude in GluN2A-TG mice. Collectively, these findings suggest that GluN2A overexpression may induce schizophrenia-like phenotypes via impairing NMDAR-dependent long-term synaptic plasticity in the PFC, likely due to altered NMDAR subunit composition leading to disrupted calcium signaling dynamics. These results provide critical insights into the pathological role of GluN2A in schizophrenia. © 2025 Elsevier Inc.

Lactate inhibits T-cell activation in sepsis through CD40LG downregulation and SOCS3-mediated JAK1/STAT3 pathway suppression

Objectives Lactate, an indicator of sepsis severity, affects kinase activity and is often associated with immunosuppression. This study investigates the impact of lactate on kinases and immune cells and identifies pivotal genes governing their interactions. Methods Differentially expressed genes (DEGs) between sepsis and control groups were screened using the limma package. The correlation between lactate-related DEGs (lrDEGs) and kinase-related DEGs (krDEGs) was then assessed. Hub genes were identified using the algorithms in CytoHubba and weighted gene co-expression network analysis (WGCNA). The aberrant immune status of sepsis patients was evaluated using xCell. The findings were ultimately validated through cellular experiments. Results In sepsis, a stronger correlation between lrDEGs and krDEGs was observed among the 998 identified DEGs compared to controls. Eight hub genes were identified through CytoHubba and WGCNA. The correlation between lrDEGs (or krDEGs) and T lymphoid cell score was particularly strong among the hub genes. CD40LG and SOCS3 were identified as key regulators of T cell function. These genes were closely associated with lrDEGs. Cellular experiments demonstrated that lactate inhibits T cell activation through downregulation of CD40LG and suppression of the SOCS3-mediated JAK1/STAT3 pathway. Conclusion Lactate-induced inhibition of T lymphocyte activation in sepsis is associated with altered expression of kinase-related genes. Elevated lactate levels downregulate CD40LG and SOCS3 expression, leading to T cell suppression by inhibiting the JAK-STAT signaling pathway.

Inpatient vs outpatient cognitive behavioral therapy for insomnia: A two-arm parallel randomized controlled trial

Objective While outpatient cognitive behavioral therapy for insomnia (CBT-I) represents the most efficacious delivery method, approximately 50 % of patients are unable to achieve clinical remission. There is a pressing need to develop more effective approaches to CBT-I delivery. This study assessed the comparative efficacy of one-week inpatient CBT-I and eight-week standard outpatient CBT-I. Methods A randomized controlled trial was conducted with 200 adults with chronic insomnia disorder. These participants were randomly allocated to either the inpatient group ( n = 100) or the outpatient group (n = 100). The inpatient group underwent a one-week intensive inpatient CBT-I followed by weekly follow-ups program. The outpatient group participated in a standard outpatient CBT-I program conducted on an individual basis over an eight-week period. The primary outcome was the insomnia severity index score at 9 weeks after the start of intervention. Linear mixed effects model was employed for the intention-to-treat analysis. Results At the 9-week assessment, the inpatient CBT-I group demonstrated a mean change in insomnia severity index score of −13.31 (95 % confidence interval [CI], −14.47 to −12.16; Cohen's d = −3.18; P < .001). The outpatient group showed a mean change of −11.14 (95 % CI, −12.30 to −9.98; Cohen's d = −2.66; P < .001). The mean difference between groups was −2.03 (95 % CI, −3.21 to −0.85; Cohen's d = −0.48; P < .001), indicating a statistically significant improvement in the inpatient group compared to the outpatient group. Conclusions This study demonstrated that inpatient CBT-I exhibited superior efficacy compared to outpatient CBT-I in alleviating insomnia severity.

Neuropathic pain (NP) is a chronic debilitating disease caused by nerve damage or various diseases, significantly impairs patients’ quality of life. Super-enhancers (SEs) are important cis-regulatory elements, but how they affect NP remains elusive. Therefore, we aim to explore the molecular mechanism by which SEs are involved in NP progression and identify potential drug candidate targets. We first established a NP model in rats, and subsequently performed H3K27ac ChIP-Seq and RNA-Seq on their spinal cord tissues to analyze the active enhancers. By integrated analysis of ChIP-seq data and RNA-seq data, we clarified a series of SE-associated genes involved in NP progression. qPCR and double immunofluorescence staining results suggested that Jmjd1c mRNA and protein levels were significantly down-regulated in the NP model. In addition, a dual-luciferase reporter assay showed that KLF15 could activate Jmjd1c transcription by binding to the SE of Jmjd1c. functionally, enhanced Jmjd1c can inhibit the levels of inflammatory cytokines such as IL-6, TNF-α, IL-1β, and inhibited the progression of NP, whereas silencing Jmjd1c had the opposite effect. Mechanistic exploration identified Jmjd1c exerted its anti-NP effect via positively regulating Socs3 expression by increasing the activity of H3K9 demethylation, and the Jmjd1c/Socs3/JAK/STAT3 regulatory pathway was finally validated as downstream effectors. In conclusion, our study suggests that SE-associated Jmjd1c was suppressed during NP progression due to the decreased recruitment of KLF15. The reduction of Jmjd1c downregulated Socs3 through the demethylation of H3K9 at Socs3 promoter region, leading to NP progression. © 2025 Chongqing Medical University
